Please understand, however, that the financial responsibility for treatment we provide remains with you. If your insurance plan is unable to make a timely payment for our services, we may request your assistance in following up with your insurance company. Please pay for all patient co-pay, co-insurance or deductible amounts at the time your treatment is provided.
